Antibody-Drug Conjugates – ADCs (Smart Chemotherapy)

What is Targeted Therapy? And how does it differ from Chemotherapy?

The core concept behind this technology is combining two types of treatment into a single compound:

  1. Targeted Antibody: Acts as a navigation system or “explorer,” precisely recognizing specific proteins found on the surface of cancer cells.

  2. Cytotoxic Payload: A highly potent anti-tumor drug attached to the antibody via a “chemical linker.”

How it works inside the body:

  • This “smart bomb” travels through the bloodstream without harming healthy cells along the way.

  • The antibody recognizes and binds exclusively to the targets on cancer cells.

  • The cancer cell internalizes the drug, where the chemical linker detaches and releases the active chemotherapy directly inside the cancer cell to destroy it from within. This significantly reduces the impact of chemotherapy on healthy body tissues.

Types of Targeted Therapy and How They Work

This therapeutic family is being used with growing success in many types of tumors, most notably:

  • Breast Cancer: Such as HER2-targeted drugs (like Trastuzumab Deruxtecan and T-DM1), and others for triple-negative breast cancer (like Sacituzumab Govitecan).

  • Bladder and Urinary Tract Tumors: Such as Enfortumab Vedotin.

  • Blood and Lymph Node Cancers (Hematologic Malignancies): Such as Brentuximab Vedotin and other treatments for B-cell lymphoma.

  • Lung Cancer and Other Types: Clinical applications are continually expanding to include new types of solid tumors.

Advantages of Antibody-Drug Conjugates Compared to Conventional Treatment

  • Targeting Precision: Concentration of most of the therapeutic substance directly inside diseased cells.

  • Higher Efficacy: Utilizing doses of tumor-killing agents that are more potent than conventional chemotherapy.

  • Fewer General Side Effects: Compared to systemic chemotherapy, effects on the rest of the body’s healthy cells are reduced (such as severe hair loss or acute impact on the digestive system).

Side Effects and Effects to Monitor

Despite their precision, they may cause some side effects that require regular monitoring with the medical team:

  • Blood Cell Counts: Such as white blood cells or platelets.

  • General Fatigue and Nausea: Usually mild to moderate in severity.

  • Drug-Specific Effects: Some medications may affect liver function, peripheral nerves (tingling in the extremities), or lung health.

Red Flags (Important Warnings Requiring Immediate Medical Contact)

Patients must be advised to contact their doctor urgently or head to the treatment center if any of the following symptoms occur:

  1. Fever (38°C or higher): Or signs of infection such as severe chills.

  2. Sudden Shortness of Breath or New/Worsening Cough: Especially with medications that may cause lung inflammation as a side effect.

  3. Unusual Bleeding or Bruising: May indicate a sharp drop in platelets.

  4. Severe Diarrhea or Persistent Vomiting: Leading to dehydration or an inability to retain fluids.

  5. Severe Numbness or Weakness in Hands and Feet: Impairing movement or walking.
